Millwork for Medical and Dental Offices

Medical and dental casework fails in ways residential cabinetry never does, and it fails fast. The environment involves hospital-grade disinfectants applied several times a day, heavy equipment loads, wet processing, strict accessibility law, and — in dental and surgical settings — infection-control requirements that dictate what a cabinet may be made of. A practice that buys residential-grade cabinetry for a clinical suite will be replacing it in three to five years.

Disinfectants are the real material test

The single biggest difference between a medical cabinet and a residential one is chemical exposure. Clinical staff wipe surfaces down with quaternary ammonium compounds, accelerated hydrogen peroxide, sodium hypochlorite (bleach), and 70% isopropyl alcohol — often several times an hour on high-touch surfaces.

What survives that:

  • High-pressure laminate (HPL) over a moisture-resistant core, with chemical-resistant grades specified where processing occurs. Proven, economical, endlessly repairable at the panel level.
  • Compact laminate (solid phenolic core). No substrate to swell, water-immune, and the correct answer for wet areas, sterilization suites, and lab benches. More expensive, essentially indestructible.
  • Thermally fused laminate (TFL) for interiors and low-exposure areas — excellent value, mechanically bonded, easy to clean.
  • Solid surface (acrylic) for countertops with integrated, coved sink bowls. Non-porous, seamless, repairable, and the reason it dominates clinical counters.

What does not survive: thermofoil (delaminates under alcohol and heat), site-applied paint, unsealed wood, and any particleboard core with unsealed edges. If a wood aesthetic is required in patient-facing areas, use a wood-look TFL or HPL — modern textured woodgrains are convincing and clean up with bleach. Real veneer belongs in the private office, not the operatory.

Infection control: the details inspectors look at

Beyond material choice, geometry matters. Surfaces must be cleanable, which means eliminating the places contamination hides:

  • Coved, integral backsplashes — no 90-degree seam between counter and wall where a sponge can't reach.
  • Integrated sink bowls thermoformed into solid surface, with no rim, no caulk joint, and no seam.
  • Full-height sealed toe-kicks, or a recessed toe with a coved base tile. In some jurisdictions, sealed-to-floor plinths are required in procedure rooms.
  • PUR-glued 2 mm edge banding on every panel edge, including hidden ones. Water and disinfectant wick into raw edges.
  • Solid, cleanable pulls — stainless wire pulls or integrated J-pulls. Avoid knobs with crevices and avoid any pull with a hollow backside.
  • Sealed penetrations at every plumbing and electrical pass-through.

ADA: the numbers that appear on every plan check

Medical offices are places of public accommodation, so the 2010 ADA Standards apply and plan reviewers check them. The ones that hit millwork:

  • Reception / transaction counters: a portion at least 36 inches long must be no higher than 36 inches above the floor, with either a parallel or forward approach. The classic 42-inch reception transaction shelf alone does not comply — you need a lowered section.
  • Forward-approach work surfaces: 28–34 inches high, with knee clearance 27 inches high, 30 inches wide, and 11–25 inches deep.
  • Lavatories: counter no higher than 34 inches; exposed pipes insulated or shielded.
  • Reach ranges: 15 inches minimum to 48 inches maximum for anything an occupant must operate — which governs upper cabinet and shelf placement in accessible exam rooms.
  • Clear floor space: 30 x 48 inches at each accessible element.

Room by room

Reception and check-in. The first impression and the ADA hot spot. Typical build: a 42-inch transaction top over a 30-inch staff work surface, with a 36-inch-long accessible lowered section. Add a knee-space workstation, a locking file bank, printer garage with ventilation, and cable management. Patient-facing panels are where a practice can spend on veneer, fluted fronts, or backlit signage without any infection-control conflict.

Exam rooms. Usually a 4–6 foot base run with an integrated sink, upper cabinets for supplies, a writing surface, and a locking drawer for samples or controlled items. Keep the sink at the door side so staff wash hands on entry. Standardize the exam-room casework across every room in the practice — it makes stocking easier and it cuts our production cost significantly.

Sterilization suite (dental). The most demanding room in the building. Design it as a directional flow: dirty receiving → ultrasonic/washer → packaging → autoclave → clean storage, with no crossing paths. Compact laminate or solid surface throughout, a deep integrated sink at the dirty end, heat-resistant surfaces beside autoclaves (this is one of the few places I specify a stainless or sintered stone landing zone), and generous power. Get the equipment cut sheets before we draw — autoclave dimensions and clearances drive the whole layout.

Operatories and treatment rooms. Rear and side delivery cabinets sized to the chair manufacturer's template. Never draw these from the architectural plan alone; get the chair and delivery-unit shop drawings, because a two-inch error puts the assistant's cart in the doctor's path.

Lab and dispensary. Chemical-resistant tops, deep base cabinets for equipment, and lockable narcotic storage where applicable — typically a steel-lined, separately keyed compartment inside a base cabinet, in line with DEA storage guidance.

Load, equipment, and power

Medical casework carries more weight than residential. Countertops supporting centrifuges, autoclaves, or imaging equipment need continuous support and, in some cases, a reinforced substrate rather than a standard cabinet top. We specify drawer systems rated to 70 lbs as standard and heavy-duty runners where instrument trays are stored; Blum and Hettich both offer rated commercial-duty options with cycle testing that matters when a drawer opens sixty times a day.

Power and data must be coordinated early. Every clinical casework run needs more receptacles than the architect drew, plus data drops for monitors and intraoral scanners, plus grommets and raceways so the cords don't sit on the counter. Retro-fitting power into finished casework doubles the cost of the change.

Procurement, lead time, and phasing

Medical fit-outs are schedule-driven — a practice that can't open on the target date is losing revenue every day. Our commercial process runs: site verification and equipment coordination → shop drawings → approval → production → delivery. Shop drawing approval is the critical path and it is where projects lose weeks, because equipment selections are often still unresolved. Production is 8–12 weeks after approval, and we can phase delivery by room so the GC can finish and turn over the front of house while clinical rooms complete.
